I acknowledge by signing this waiver that I have been given the full opportunity to ask any and all questions I might have about obtaining a tattoo from artist listed below, hereafter to as the studio. I acknowledge that all my questions have been answered to my full and total satisfaction. I especially acknowledge that I have been advised of the matter set forth below, and the technician shall not perform a body art procedure if the client fails to complete or sign the disclosure and authorization form, the technician may decline to perform a body art procedure if the client has any identified health conditions.

PLEASE CHECK:

I am eighteen years of age or older, and that I'm obtaining a tattoo by my choice alone.

I accept all risks of injury both known and unknown as a result of having a part of my body tattooed and I hereby consent to being tattooed.

I understand that by being tattooed I am making a permanent change to my appearance.

I understand that allergic reactions to the dyes, inks, pigments, ointments, sprays, adhesives, and other processes or equipment used to apply and bandage my tattoo are always possible - and accept the risks of any and all allergic reactions as a result of being tattooed.

I understand that tattooing is not perfect - and I accept the risk that my tattoo may vary in size, shape, location, color, shading, spelling, lettering, or meaning, other than what I had expected or requested.

I accept the risk of infection, scarring and permanent injury as a result of being tattooed.

I accept the fact that no guarantee is made or implied as the quality of final outcome of my tattoo.

I understand and accept the risk that it may be impossible to ever change, alter, or remove my tattoo.

I acknowledge that the proprietor(s), management, and tattoo artist reserve the right to refuse service to anyone FOR ANY REASON.
